Leadership withheld key information about the incident from other employees in the same role and ignored repeated requests for basic protections like shoulder seatbelts. Similar near-misses occurred with vehicles almost colliding with oncoming box trucks due to unsafe software. * Toxic Culture: A fear-driven management style where employees are constantly pressured with tasks outside their roles and expected to “do more” without support. * Training Issues: Extended, unclear training processes with little accountability.
